Coverage Eligibility Screening Appointments

This appointment is for individuals and families who want help understanding their health coverage options or who need help managing existing coverage. If you’re unsure what coverage you have—or what coverage you may be eligible for—this is the right place to start.

Preparing for Your Appointment

What to Expect:

During your appointment, a PANOW navigator will review your situation, explain your options, and help you take the next steps, whether that’s enrolling, renewing, or responding to a notice you’ve received. Please bring the following items if you have them or if they apply to you:

  • Insurance letters or notices
  • Proof of income
  • Household information
  • Immigration or citizenship documents

We can help with:

  • Eligibility screening to determine what coverage you may qualify for

  • New health coverage applications
  • Renewals and redeterminations
  • Responding to letters or Requests for Information (RFIs)
  • Reporting changes (income, address, household size, employment, etc.)
  • Terminating coverage, when needed
  • Transitioning between coverage types, including:

    • Moving from OHP (Medicaid) to Marketplace coverage
    • Moving from Marketplace coverage to OHP

Coverage and insurance types we support:

  • Other possible public or private coverage options

  • External referrals to support for Medicare

Support for Community Partners and Organizations

PANOW also works with community partners to strengthen access to health coverage. We can:

  • Help organizations set up referral pathways for clients who need insurance assistance
  • Provide training sessions or group presentations on health insurance options, including OHP, Marketplace coverage, and coverage transitions
